Who we are
Hawthorn is operated by Hawthorn Health Ltd from 14 Church Lane, Bristol BS6 5TB. We are the data controller for the personal information we hold about you. The pharmacy is registered with the General Pharmaceutical Council (GPhC) under number 1123456. Our superintendent pharmacist is Priya Sharma MRPharmS.
What we collect
- Your name, date of birth, address, email and phone number
- NHS number and GP surgery, where relevant
- Records of prescriptions, services and consultations we provide
- Allergies, medical history and other clinical information you share with us
- Booking and nomination forms submitted through this website
Why we use it
We use your information to provide pharmacy services safely, dispense NHS and private prescriptions, contact you about appointments, and meet our legal and regulatory obligations under the Human Medicines Regulations 2012 and NHS terms of service.
Who we share with
With your consent we share information with your GP surgery, NHS Business Services Authority, and (where clinically necessary) secondary care providers. We do not sell personal information to third parties.
Your rights
Under UK GDPR you have the right to access your data, correct it, ask us to delete it where we are not legally required to keep it, and complain to the Information Commissioner's Office. To exercise any of these rights, email [email protected].
